Thank you for your interest in our business lending solutions! The following is provided to assist you through the process of gathering the necessary information for the evaluation of your loan request(s). Complete information will ensure timely evaluation of your request. Please provide the following documents (when applicable):

  • Business Loan Application
  • Personal Financial Statement
  • Last three years of completed Business Tax Returns containing including ALL schedules
    (2 years for aggregate loan balance(s) under $50K)
  • Last three years of Business Financial Statements
    (2 years for aggregate loan balance(s) under $50K)
  • Income Statements (or "Profit and Loss Statement")
  • Balance Sheets
  • Last three years of Personal Federal Tax Returns containing all schedules for all guarantors and State Tax Returns, including ALL schedules, for each natural person (2 years for aggregate loan balance(s) under $50K)
  • Copy of Partnership Agreement, Articles of Incorporation or Articles of Organization, if applicable
  • Purchase order/purchase agreement for collateral being acquired

After photocopying your financial statements and tax returns, please affix them to your completed application and drop off at a branch location or fax to 651-747-1755.
